· Clubs will run concurrent with the school calendar. No clubs on half days.

· I understand it is my responsibility to have my child picked up at the end of Club, or have him/her signed up for ASP.

· I understand that no IGNITE 3:16 Club volunteer may provide transportation for my child without prior, written approval

At IGNITE 3:16, we will do everything we can to be compliant with all regulations and ensure your/your child’s safety. We have put in place measures to reduce the spread, but cannot guarantee that you/your child will not become infected with COVID-19.

COVID-19 Waiver: With full knowledge of the risks involved, I hereby release, waive, discharge Club 3:16, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me/my child related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.
